 Northstar Physicians Network has opportunities for RNs, LSWs, LPNs and RRAs: Challenging and exciting full time and part time opportunities in our Clinical Operations area. We seek individuals with long term care experience, and/or care coordination/case management and utilization review experience (though we will train the right candidates). Positions may also work within our Quality Improvement initiatives.

RNs, LPNs, or RRAs are also encouraged to apply for a position working in support of our QI, Disease Management and Clinical Operations areas. 

All positions involve contact with a variety of areas including:

• patients
• patients families
• physicians and providers
• health plans
• community resources

Requirements:

• Valid Minnesota RN license / or social worker license
• 2-3 years experience in long term care, care coordination and/or utilization review. We will train the right candidate.
• Computer skills (Microsoft Office products)
• Professional manner with ability to interact with our partners and be a strong advocate on our patients behalf.
